These Terms of Service ("Terms") govern your access to and use of the Cortex Neurovascular website (cortexneurovascular.com), the TEMMA Patient Companion App ("Patient App"), and the Cortex Referral platform ("Referral App"), collectively the "Services." The TEMMA Patient Companion App is designed to help patients track their treatment journey, log headache symptoms, and receive appointment reminders. The Patient App is a companion tool and does not provide medical advice, diagnosis, or treatment recommendations.
The Cortex Referral platform allows referring physicians to submit patient referrals to Cortex Neurovascular. By using the Referral App, you represent and warrant that you are an authorized healthcare provider or staff member submitting referrals on behalf of your practice. You are responsible for the accuracy of the information submitted and for obtaining any necessary patient consent prior to referral.
The Services, including all content, text, graphics, logos, images, and software, are the property of Cortex Neurovascular and are protected by United States and international intellectual property laws. You may not reproduce, modify, distribute, or otherwise use any content from the Services without prior written consent.
"TEMMA," "Cortex Neurovascular," and associated marks are trademarks of Cortex Neurovascular. All other trademarks, service marks, and trade names are the property of their respective owners.
The Services are provided "as is" and "as available" without warranties of any kind, whether express or implied. Cortex Neurovascular does not warrant that the Services will be uninterrupted, error-free, or secure, or that any information provided through the Services is accurate, complete, or current.
To the fullest extent permitted by law, Cortex Neurovascular shall not be liable for any direct, indirect, incidental, consequential, special, or exemplary damages arising from your use of or inability to use the Services, including but not limited to damages for loss of profits, data, or other intangible losses, even if we have been advised of the possibility of such damages.
The Services do not constitute medical advice. Any health-related decisions made based on information obtained from the Services are solely at your own risk. Always consult a qualified healthcare provider for medical advice and treatment.
We reserve the right to modify these Terms at any time. If we make material changes, we will update the "Effective Date" at the top of this page and, where practicable, notify users through the Services. Your continued use of the Services after any changes constitutes acceptance of the revised Terms.
We may suspend or terminate your access to the Services at any time, without notice, for any reason, including if we believe you have violated these Terms. Upon termination, all licenses granted to you will immediately cease.
